American Senior Communities Physicians Discharge Program supports individuals who are preparing to go home.
"There is a gap sometimes when individuals go home but they don't have a primary care physician or they haven't seen one for several years and it delays the gap in their care when they go home," said Troy Reiff, Vice President of Post Acute Services at American Senior Communities.
"We identified that problem and put together a team like Dr. Benfield at Meridian Visiting Physicians and others around the state of Indiana to support and allow those individuals to be seen in a timelier fashion when they go home," Reiff said.
Dr, Benfield with Meridian Visiting Physicians says they come into the facility and gather information so they can coordinate care for the resident so that when they get home they have all of the things they need.
"Once they are home we send a team out to their home and provide primary care," said Dr. Benfield. "We are a home based Primary Care practice that will provide care to them when they are home."
